• must have diagnosed qualifying medical condition with verifying recent documentation including prior physician evaluations, laboratory results, imaging (x-rays, CT scans, MRI)

• undergo full medical evaluation for $200 at time of evaluation

• continue to follow up for scheduled visits in order to maintain eligibility​
